 A 79 year old female with a history of RA and parkinson's dx presents to the ED with a syncopal episode. She fell from a standing height and hit the back of her head. She was unable to stand and called EMS via medical alert signal when she awoke. She is complaining of midline neck and head pain that is worse when she rotates her neck. She does not have any focal deficits. She was not intoxicated. She has normal mental status. GCS= 15. She has pain when palpating midline C2-C4. She does not have any long bone injuries, but has rib pain and tenderness on the left side. Being an astute ED resident you order a CT of the neck. What do you see? (hint look at C2)

 A 30 year old male presents to the ED after injuring his left knee when playing basketball. He says that he went up for a rebound and came down on his knee. He heard a pop, felt severe pain and was not able to walk since. On exam you notice significant effusion of the knee, he is unable to flex his hip and hold his leg up in the air. Even with assistance he has difficulty raising his leg and his lower leg falls to the bed. There is also a step-off along the pattelar tendon. The knee is stable and he has no tenderness over the bone. Below is his xray. What is your diagnosis and management?

 A 12 year old female presents to the ED with a rash over her hands, lower extremity, and face and lips. The patient recently had a URI and was treated by her pediatrician with Azithromycin. Her URI symptoms resolved 2 days after starting the medication. She then developed the rash. She has not eaten after getting the rash because of pain in the mouth. The rash is not itchy. There are multiple lesions on lips, none in the vaginal, rectal or ocular areas.  No one else has a similar rash in her family and she has not traveled outside of the country. She has no other medical problems. What is your diagnosis?

A 56 year old female presents to the emergency department after a fall backwards on a stretched hand. The patient presents with tremendous pain in the right arm. She is unable to fully extend the elbow and has much discomfort when asked to actively flex the elbow. She cannot supinate or pronate. Upon palpation you notice a visible deformity and effusion of the elbow. The patient has a radial and ulnar pulse. The extremity is warm. The patient has normal sensation along the ulnar nerve distribution. Describe the patient's elbow x-ray.

A 33 y/o female presents to the ED with 2 hours of heavy vaginal bleeding and abdominal pain. She has a near syncopal episode in triage where her HR dropped from 110 to the 50's. She has a positive pregnancy test in the ED. She has no medical problems; and no complications during her previous pregnancies. Her abdomen is diffusely tender with rebound and guarding of the LLQ. The following is an image of the TVUS preformed in the ED. What was her cause of near syncope and bradycardia?
